110-300-0165(2)(c) Licensee had cord accessible from a lap top computer on the kitchen counter, dog bed warmer bed cord in the living room, two vacuum cleansers in play room area with cords, toothbrush charger with a cords on the bathroom sink, that all are capable of forming a loop around a child’s neck. 110-3000165(2)(d) Licensee had Walmart plastic bags accessible in the an unlocked cabinet in the small play room by the bathroom and a large black garbage bag in the play room by the bathroom that are suffocation hazards. 110-300-0165(3)(f) Licensee had a large white cabinet used for the children’s cubbies in the play room that is six shelves high that was not secured to the wall posing a tipping hazard.
